This is a legal document from Form DSCB:15-8821. Prerequisite Steps To Take Before Using This Form

When you look at this form, you are going to notice that several things must be done prior to filling it out. That is because without taking these steps, the information they are requesting cannot be put in. An example of this would be the name of the LLC that you would like to set up. You must first use the business name database from the Pennsylvania Department of State, to see if someone has already taken it. If they have not, you will then need to fill out and submit Form DSCB:15-208. This is the name reservation form. In order to reserve the name, and ultimately have it for your LLC, it will cost $70 for the filing fee.

Next, finding a registered agent would be the next step. Every LLC in Pennsylvania must have an agent for the service of the process. When the state sends you papers regarding legal issues, specifically lawsuits, it needs to be sent to this person or organization. They must have a physical address where mail can be sent to qualify. Completing Your Certificate Of Organization

To complete this form, you will then add in your LLC name, the name, and the address of the registered agent, and then you will add the rest of the information. This is divided up into two separate forms. The first form, Form DSCB:15-8821, will contain not only the name of the LLC and the registered agent, but you must also list each organizer, the effective date of the certificate, and what the LLC will be used for.

You must also have the signatures of the organizers at the bottom. The second document is Form DSCB:15-134A. This is called the New Entity Docketing Statement. Again, your LLC name will be added to the top. However, you will now need to list the person responsible for taxes, the LLC’s business activity, the EIN number, and you will put the month of December for the fiscal year-end. Once this is done, you can submit this by mail or online in order to have it approved.

Once you have completed this process, you will soon have your LLC approved and active. In most cases, it will take between four and six weeks which is standard.
